GaHIN has an incredible membership group that meets once every quarter. This quarter’s meeting was jam-packed with updates on some of our most exciting news — everything from launching our new integrated clinical and social care system to new members and new capabilities. This blog attempts to summarize the top five takeaways from that meeting.
1. Enabling Whole-Person Care through GaHIN’s Integrated Platform
GaHIN enables whole-person care through a single platform, bridging the gap between clinicians and social care workers to create a longitudinal patient record. Our platform integrates clinical, social, and behavioral health data. We announced the availability of housing data populated from Georgia’s Department of Community Affairs’ HIMS client roster. The data flows into GaHIN’s Central Data Repository and can be matched with clinical data to inform authorized clinicians if a patient has requested housing assistance in the past.
2. New Capabilities Available from GeorgiaConnX
GeorgiaConnX, GaHIN’s clinical data exchange platform, now enables users to create cohorts of patients who may need specialized care services. Users can also access GaHIN’s Community Resource Directory either through the Clinical Viewer or their EHR system. They can electronically send referrals directly through the system to community-based service organizations to help patients access the services they need. Plus, the referral source is automatically notified once the referral is accepted, creating a closed-loop process.
3. Member Training and Support Resources:
We are proud to announce the availability of new end-user training resources, including a GeorgiaConnX Reference Guide and live training sessions. The training includes virtual role-based training, train-the-trainer sessions, FAQs, and a Help Desk. With more than 70% of our members live on the new platform, these resources will be important tools for users who want to get up to speed quickly on GaHIN’s newest capabilities.
4. GaHIN’s Social Care Strategy, GeorgiaUnify is Live
Members have heard us talk about GaHIN’s new social care platform for some time, and we are pleased to announce that GeorgiaUnify is now live and available for all members. The solution captures and securely exchanges social care data, similar to how GeorgiaConnX captures and shares clinical data. GeorgiaUnify data can also be matched with clinical data in GeorgiaConnX for providers who want a more complete picture of their patients.
One of the most exciting elements of GeorgiaUnify is its ability to connect providers and patients with a robust Community Resource Directory that is being populated by the Georgia Department of Community Affairs’ Housing Resource directory. It will soon be populated by United Way’s 211 directory, which contains more than 4,000 community-based service organizations.
5. GaHIN By the Numbers
With more than 3.7M unique patient records, GaHIN’s coverage of Georgia lives is growing rapidly, and so is the volume of data that we are making available to our members. Here are some of the latest statistics on data that is flowing through GaHIN’s Central Data Repository.
